Internal Piles are always painless. Payal are nothing but abnormally, dilated and swollen veins. internal Piles are covered by mucosal skin over bulge, veins . Internal piles bulge at inside and causes obstruction to pass the stool. But as they are covered with elastic mucosa skin, they can get adjust with size due to pressure caused by passing stool. Hence, there is no pain at all.
Patient may realise little discomfort or sometimes bleeding may happens, which is painless.

3rd°, internal haemorrhoids piles- Payal mass comes out of the anus, and Patient has to push it back with finger after passing stool.
4th°, internal haemorrhoids- piles - the permanently remains outside.
Patient feels itching, discomfort, constipation, or occasional bleeding. Most of the patient ignores the symptoms.
But some may land up in complications.
